Your portfolio should contain knowledge of evidence-based practice

All applicants must utilise the “All Wales AC Portfolio Guidance” [Add guidance or link to page with guidance] document and complete all associated templates

  • All cases must be fully anonymised (no patient identifiable information; this includes names of staff, names of wards and care homes etc), and where appropriate, signed and dated testimonials from the AC/RC(s) you have shadowed must be included in the portfolio.
  • You should ensure that your evidence is supported and signed off by a variety of practitioners other than the AC/RC you have shadowed.
  • Applicants must ensure all evidence is clearly numbered in the portfolio and is cross-referenced to the 9 Wales AC Competencies.
