Portfolio Checklist

• A cover page with your name, address and contact details
• A table of contents that lists the titles of the competencies within the portfolio.
• Cross-referencing where one piece of evidence satisfies a number of competencies.
• A typed index listing all of the evidence and page numbered
• The evidence itself, each item being numbered. Please ensure that you do not staple documents and that patient and staff identifiable information is removed.
• A personal statement confirming that you meet the professional requirements and that the evidence is original, authentic and has been compiled by you. To be authorised by your Senior Manager.
• A copy of your attendance certificate at the initial Approved Clinician training for Wales.
• Two references, one of which is from your present employer Support from your employing organisation in writing.
• Agreement from your employing organisation that you will have the opportunity to work as an Approved/Responsible Clinician
